The HSE has apologised for failings in their care of a 38-year-old man from Mallow who took his own life in a psychiatric hospital, hours after his admission.

An inquest was held yesterday into the death of father-of-two Anthony Hennessy at St. Stephen's Hospital in Glanmire close to 6 years ago.

The HSE apologised to Anthony's family and said it will endeavour to ensure lessons are learned for failings in the care afforded to him on November 24th 2019.

His brother Kenneth Hennessy told the Neil Prendeville show the inquest into his brothers death came too late for some of his family:

"My mum died 3 years ago, she never got to see the apology yesterday. His wife and his kids are traumatised from it like, it's after leaving a massive whole in our family."
