The Beacon Hospital's vaccination model, which was in place when 20 private school teachers were jabbed, went against HSE Clinical Guidance.
A review has found it had two main weaknesses, with one being that doses were drawn into syringes before patients showed up for their vaccine.
Hospital staff believed this meant the dose had to be used within one hour - with 20 surplus vaccines offered to teachers at St Gerard's in Bray Co. Wicklow as a result.
A review found there was no evidence to support the one hour usage claim, and that it would have been feasible for the hospital to offer the jabs to higher priority groups.
People Before Profit TD Paul Murphy says the report is very damning.
